Outpatient Servicel
I. Classification of outpatient service:
The outpatient service
of the Hospital is divided into general outpatient service and
special outpatient service.
(1) General
Outpatient Service: the part of the outpatient service timetable in
white, in which the patient can make an appointment him/herself up
to 5 weeks in advance.
(2) Special
Outpatient Service: the part of the outpatient service timetable in
red, in which the patient cannot make an appointment by him/herself,
but has to be referred by a general outpatient doctor or by a doctor
from another hospital.
II. Methods for Registering Appointment:
(1) On-site
registration: register at the registration counter within the
registration opening time period on the required day.
(2) Telephone
registration: there are staff and voice appointment lines available
to handle appointment registration for general outpatient service
within five weeks.
(3) Internet
registration: this is limited to returning patients of the Hospital
only, 24 hours, open all year, for appointment registration for
general outpatient service within five weeks.

(4) Registration
machine: placed beside the cashier/registration counter on the 1st
floor of the outpatient building and inpatient building of the
Hospital, with the following functions:
1. Make appointment
and appointment inquiry
2. Cancel
appointment
3. Inquire regarding
the queue progress
4. Inquire regarding
the availability of appointment/doctors or locum tenens (temporary
substitutes)
5. Inquire regarding
the leave time of a particular doctor
6. Inquire regarding
the status of application for medical certificate (Disability
Determination Certificate,
Disability Certificate
for Public Servant/Labor/Farmer Insurance, Serviceman Re-examination
Certificate)
III. Registration Opening Time:
(1) On-site
registration: 8:10-11:30 AM; 1:00-4:00 PM
(2) Appointment
registration: the appointment registration for general outpatient
service is open 5 weeks before the scheduled time. Appointment
registration for outpatient service on the day closes 10:30AM for
the morning session and 3:00PM for the afternoon session.

(2) Do not take up
a queue spot with your belongings. For fairness to people on-site,
those holding a queue spot with personal belongings will be deemed
as invalid.
(3) Personal
belongings in the waiting area will be cleared away at 6:10AM for
the morning session and 11:00AM for the afternoon session. The
Hospital will not be responsible for safekeeping them.
(4) Each person can
only take one number and should arrive at the registration counter
to wait for your number to be called when registration begins. If
your number has already been called when you are not there, please
inform the personnel behind the counter. If you missed the number by
30 numbers, you will have to take a new number.
(5) During the
waiting time between obtaining a number and waiting for registration
to begin (7:10 - 8:10 AM; 12:00 – 13:00 PM), you are welcome to
utilize our food shops/restaurants on B1 of the in-patient building
or on the first floor of the outpatient building.
(6) To shorten your
waiting time, you are advised to utilize our telephone or Internet
registration service.
(7) During the
access control time of the outpatient building, please enter and
exit the building from the side gate on Shengli Road.
(8) For those who
have already registered, to change to on-site registration, you will
need to cancel your previous registration first; cancellation of
registration for morning session should be made before 7:00AM of the
day’s appointment, and before 12:00 PM noon for the afternoon
session of the day’s appointment.
(9) Registration
for appointments in the in-patient building is limited to diagnosing
departments (Rehabilitation Department and Radiation Oncology
Department) in the in-patient building only.
V. Notes for registration and attending a scheduled appointment
(1) Patients on
first-time visit should register at the cashier/registration counter
with their NHI IC card and personal ID card, and attend the schedule
appointment after registration.
(2) Returning
patients
who
have completed registration should report to the doctor’s office
directly with their NHI IC card and personal ID card.
If you
do not have a NHI IC card or are receiving treatment under
self-payment, please fill in the
Reporting
Form at the waiting area, or drop the Registered Appointment Form
through the small window of the doctors’ office.
1. Those with
special NHI ID (e.g., Catastrophic Illness Card,
referral note, return
visit note, occupational injury certificate or disabled
handbook) should present it to the medical personnel in the office
for priority treatment.
2. On Saturdays,
all cashier/registration affairs of the outpatient building are
handled at the cashier/registration counter on the first floor.
(3) In case of two
defaults (missing appointment) within half a year, advance
registration for appointment will not be accepted within 3 months
from the second default. To cancel the default record, you will need
to make an on-site registration
once.
(4) Time for
canceling a registered appointment:
Morning session:
before 7:00 AM
of
the day of appointment
Afternoon session:
before 12:00 PM noon of the day of appointment.
(5) Referral
patients should present the referral note (in duplicate) and
register at any registration counter. Those with an appointment or
examination in the in-patient building can register or pay at the
outpatient counter on the first floor of the in-patient building, or
at the emergency counter on Saturdays.
(6) At the on-site
registration counter, you can only register for the morning or
afternoon session during the respective registration time (i.e., you
cannot register for the afternoon session during the registration
time for the morning session); however, appointments can be made for
doctors who are still available.
(7) After seeing
the doctor, please pay your bill at the cashier counter regardless
of whether or not a doctor’s prescription is issued. If you have
visited two or more departments, remember to pay all bills. Bills
not paid on the day of appointment will be listed as debt from the
next day.
(8) Medicine
prescribed in the outpatient department should be collected within 3
days (including holidays).
(9) In case of a
non-working typhoon day as announced by the Tainan City Government,
the Hospital will reduce the outpatient schedule.
(10) Inquiry
regarding the queue progress:
You can inquire
regarding the queue progress of the day via the Hospital’s website
or the Automatic Registration Machine.

(2) Payment and
collection of continuous prescription for chronic disease
1. Process the
medicine collection procedure at the Hospital 7 days prior to the
collection date with your medicine card and NHI IC card; if you have
missed the medicine collection date, please contact the
cashier/registration counter (extension number 4070-4080) to confirm
whether your medicine can be picked up.
2. Medicine
collection time: 7:40-19:30 Monday to Friday; 7:50-12:30 Saturday.
Closed during nighttime, holidays and national holidays.
3. Medicine
collection procedure: pay at the cashier counter of the outpatient
building first, and then go to the pharmacy counter to collect the
medicine according to the handwritten number on the medicine card.
4. In case of any
change to the health condition, please see the doctor immediately.
5. If the medicine
card is lost or has expired, see the doctor for a new prescription.
(3)
Examination
1. Examination List
(e.g., CT, NMR, ultrasound, exercise
electrocardiogram, EEG) will be prescribed on the day of the
appointment by the doctor.
2. On the day set
for examination, besides reporting to the location as stated in the
Reporting Time and Place on the Examination List, remember to bring
your NHI card.
3. Those with
appointment or examination in the in-patient building can process
the registration or payment at the outpatient counter on the first
floor of the in-patient building, or the emergency
cashier/registration counter on Saturdays.
4. Before
examination, please carefully read the instructions and important
notices issued.
(4) Obtaining the
examination report
1. Register to see
the doctor:
The report will
be interpreted and explained by the doctor in person (a registration
fee and diagnostic fee will apply.)
2. Apply for
printing the report at the service counter:
You can print
out the report for examination done within 6 months for NT$5 per
item (without doctor’s interpretation and explanation); the ID card
of the patient (and the respective consignee, if not the patient
him/herself) will be needed upon application.
3. Have the report
posted:
If the doctor
assesses that the report can be posted to the patient, the replying
fee and mailing fee will be charged to the bill of the day of the
outpatient visit; the report will be sent to the patient by
registered mail.
4. Copy
X-ray scan or image CD:
Please apply at the
counter of the Department of Diagnostic Radiology with the ID card
of the patient (and the respective consignee, if not the patient
him/herself).
Hospitalization
I. Admission
procedure:
(1)
If the outpatient
doctor or the emergency doctor
considers
it necessary for the patient to be hospitalized according to his/her
health condition,
the doctor
will prescribe a hospitalization
permission
note. Please
present
the note
to the referral center of the outpatient building, comprehensive
counter of the in-patient building or emergency cashier counter to
make a hospitalization appointment and then wait for the Hospital’s
notice at home.
(2)
Please bring the
NHI
IC card and relevant certificates (e.g.,
catastrophic illness or occupational injury certificate) on the day
to be hospitalized. Please supplement any missing documents within 3
days.
II. Discharge
procedure:
After the doctor
issues the discharge note, bring it as well as relevant certificates
and money to the pharmacy on the first floor of the in-patient
building to collect the medicine, and then go to the payment counter
of the Medical Affairs Office (first floor of the in-patient
building) to pay the fees.
